Spending quality time with each new patient is our highest priority. At Sensitive Care Cosmetic and Family Dentistry, we understand that no two smiles are alike, so we take our time to get to know you before we discuss your dental concerns. We would also like to hear about any negative experiences you have had in the past or anxiety about your trip to the office in order to do everything we can to put you at ease. During your phone call, we would also like to know about any medical issues which may affect your dental treatment as well as the times that are most convenient for your schedule. At this time, you may also select how you would prefer to be contacted by our friendly office staff including via email, text or phone call. The services we offer our Milford and New Haven, CT patients are second-to-none, and require a high level of cooperation.

Your first experience with our office will be different from any other office you may have contacted. Our goal is to tailor your visit specifically to your needs. You will be asked a series of questions, your answers will allow our team to customize your appointment to meet your needs.
Following your conversation, we will mail you your New Patient Paperwork that includes:

• Health History
• Insurance Information
• Consent for Treatment
• Map to Office
• HIPAA Policy
• Self Addressed Stamped Envelope

We will ask you to complete the necessary information and mail the forms back to our office in the envelope provided so we may have them two days prior to your dental appointment. Receipt of these forms will confirm your reservation.
